ROCK BRIDGE WRESTLING CLUB NOTIFICATION OF INHERENT RISKS
Wrestling is an exciting contact sport that involves physical contact between two athletes. Because of these conditions inherent to the sport, participating in wrestling exposes an athlete to many risks of injury. Those injuries include, but are not limited to, death; paralysis due to serious neck and back injuries; brain damage; damage to internal organs; serious injuries to bones, ligaments, joints, and tendons; and general deterioration of health. Such injuries can result not only in temporary loss of function, but also in serious impairment of future physical, psychological, and social abilities, including the ability to earn a living.
In an effort to make the sport of wrestling as safe as it can be, the coaching staff will instruct wrestlers concerning the rules of wrestling and the correct mechanics of all skills. It is vital that athletes follow the coach's skill instructions, training rules, and team policies to decrease the possibility of serious injury. Team rules and policies are listed in the Wrestler packet each athlete receives.
We have read the information above concerning the risks of wrestling. We understand and assume all risks associated with practice and matches. We further agree to hold Rock Bridge Wrestling Club and its representatives, coaches, volunteers, Rock Bridge High School, Columbia Public School District, and agents harmless in any and all liability actions, claims, or additional legal action in connection with participation in any activities related to the participation on the Rock Bridge Wrestling Club.
In signing this form, we (the parent(s)/guardian(s) assume the inherent risks of wrestling and waive future legal action by our heirs, estate, executor, administrator, assignees, family members, and ourselves.
Note: The athlete and both parents or legal guardians (if living) must sign this form before athlete may participate in sport practices or matches. If one parent or guardian is deceased, please indicate so on the appropriate line.
